The use of preparation CRATAL

        The experience of use of drugs of vegetal origin in the clinic of internal diseases proves their efficiency, in particular in treatment of cardiovascular disorders. Vegetal preparations have some advantages over synthetic ones, as they very rare show adverse side effects such as allergic response and are well tolerable. Preparation Cratal is one of new pharmacological preparations used in cardiology. It had been developed by State scientific center of drugs and is manufactured by SPC “Borschagovsky CPP”. Every 1 g package of granulated Cratal and every 1 g tablet contain 0.043 g thick extract of Crataegus oxycante, 0.87 g thick extract of Leonurus cardiaca and 0.867 g taurine. In both extracts flavonoids and their glycosides prevail as well as organic compounds of various chemical structure, however pharmacological effect of the majority of them has a directed cardiotropic character. It is just the complex of biologically active substances of the extract, which results in synergism of therapeutic effects of its separate components. Taurine –protein-free sulfur-containing/3-amino acid - amino ethane sulfonic acid, having inotropic action on cardiac muscle and participating in regulation of cardiomyocytes metabolism, is a significant component of Cratal. Taurine directly participates in metabolic processes of carbohydrate and proteins exchange and has a membrane-stabilizing effect due to inhibition of lipids peroxidation (LP). Cardioprotecting effect of taurine was ascertained in experiments for the study of its effect on functional status of cardiovascular system with ischemic and reperfusive myocardium troubles. During the study significant anti-arrhythmic and hypotensive of taurine and its important role in exchange of ions Na4, K4 and Ca21 in cardiomyocytes with cardiac muscle overload, caused by activation of sympathoadrenal system, were defined. Specific chemical features of taurine condition its unique effect as a universal modulator of the level of cytoplasmatic Ca24 and intracellular “calcium agonist”. It was experimentally proved that cardioprotecting properties of taurine are conditioned by its impact on secondary messengers and on hemic hypoxia processes, i.e. that it has a direct antioxidant activity. Taurine also participates in bile acids and cholesterin exchange with formation of cholyltaurine, which fact conditions probably its lipidemic properties. Inhibiting effect on rennin-angiotensin and callikreinkynin system, beneficial action on cyclo-AMP production and inhibition of lipids peroxidation are leading mechanisms of preparation Cratal. Thus, Cratal gives an expressed inhibition of activity of rennin, which transforms angiotensin into angiotensin I – a predecessor of angiotensin II, which is one most powerful vasoconstrictors. Materials and methods Two groups of 50 male patients having average age 42.7 + 5.3 years were examined. The first group included 36 patients with cardiac ischemia with soft arterial hypertension (AH) and the 2-second one included 14 patients with neurocirculatory dystonia (NCD). All patients with NCD were treated only with Cratal – 1 dose 3 times a day within 10 to 20 days, other patients used the preparation on the background of a basic therapy (antagonists of Ca2”1”, beta-adrenoblockators, nitrates and APF inhibitors). In the process of examination arterial pressure and heart rate were monitored, values of double product (DP) before and after therapy were calculated, indicators of cardiodynamics were defined by echocardiograms and tolerance to dosed physical load (DPL) was defined by method of paired ergometric tests. Levels of electrolytes in plasma, of uric acid, cholesterin, triglycerides and ferments alanine amino transferase and aspartate amino transferase were analyzed. Preparation efficiency was evaluated using point scale: very good effect (1 point), OK (2 points), satisfactory (3 points) and bad (4 points). The obtained results were processed using methods of variation statistics and package of applied statistical programs. Results and discussions Therapy using preparation Cratal within 10 days has resulted in reduction of systole and diastole arterial tension with further its stabilization as well as heart rate and DP reduction, which demonstrates more saving work of myocardium and it less need in oxygen with the same physical activity of the patient. The analysis of dynamics of diastole arterial tension has shown, that average value of the indicator in the group reduced by 4.4 + 1.1 mm Hg, individual changes varying from +2 to –10 mm Hg.
