Effect of INDOPRESS (indatamide)

          In accordance with recommendation of WHO (1999) and of the US National Committee for revelation and treatment of arterial hypertension (1997) diuretics are leading in treatment of arterial hypertension (AT). However, in line with favorable properties diuretics have unwanted ones such as negative effect, limiting their use: adverse effect on metabolism of carbohydrates and lipids, potential reflex stimulation of rennin-angiotension and sympatho-adrenal systems, reduction of potassium content in blood plasma and increase of uric acid. In this context implementation of diuretics, which do not affect metabolism and blood pressure systems, into medical practice is of great importance. Nowadays, when prescribing anti-hypertension therapy, special attention is drawn to AT circumdaily rhythms: medicines, having stable and smooth anti-hypertension effect without sharp AT variation and reducing its daily variation, are optimal preparations. Indopress (Indapamide) made by Borschagovsky CPP, was clinically tested in hypertension department of the Institute of cardiology named by N.D. Strazhesko of AMS of Ukraine in order to clinically evaluate the preparation and to study its effect on metabolic parameters and blood system indicators. 30 patients with hypertension at stage II aged from 29 to 56 years were examined. Reduction of diastolic arterial pressure (DAT) down to the level lower than 90 mm Hg of by 10% of initial pressure was taken as a criterion of the efficiency of therapy with the use of Indopress.. AT was daily monitored using preparation BR-102 made by company Shiller: in daytime – every 15 min, at night (from 10 pm to 7 am) – every 30 min. Values and rate of AT rise in the morning (a difference between maximal and minimal AT values from 6 am to 9 am), minimal/maximal (T/P) index characterizing preparation anti-hypertension efficiency and longevity of its action were also studied. The use of Indopress in daily dose 2.5 mg within 4 weeks proved its high anti-hypertension efficiency. AT normalization was achieved in 57% patients, DAT reduction by more than 10% - in 18% patients. Good tolerance of the preparation was marked with no side effects. All patients noted subjective improvement proved by reduction of recurrence of headache attacks in the process of therapy from 85% to 14%, of vertigo – from 69% to 7% and heart pain – from 65% to 8% cases. In the majority of patients complaints on undue fatigability and weakness have reduced. In 2 weeks of therapy true reduction of average daily AT (SAT) and DAT was marked: SAT reduced from 154 + 5 to 138+3 mm Hg. (р<0.01). In 4 weeks of therapy anti-hypertension effect of Indopress intensified: SAT and DAT reduced within all periods under study and their normal daily characters revived more significantly. Upon 4 weeks of therapy by Indopress residual anti-hypertension effect of SAT was 60 + 4% and of DAT – 59.4%, which fact proves the preparation capability to control AT within 24 hrs a day with a single uptake. This property of Indapamide results in its capability to reduce AT rise in the morning and its rate. At the end of the therapy course the value of AT rise in morning reduced by 36% for SAT and by 35% for DAT and its rise rate reduced by 37% and 17% correspondingly. It is known that diuretics effect on pressure humoral systems is one of their adverse effects. In this context we studied Indopress effect on the content of catecholamines (adrenalin and noradrenalin) and activity of APF. The obtained results have shown that Indopress does not cause activation of the sympathoadrenal system, which is of a great importance for insurance of its anti-hypertension effect. Under the effect of therapy activity of the ferment, forming angiotensin II, also essentially reduced and its growth from 6 am till 9 am dropped. In our study Indopress neither affected glucose level in blood on empty stomach nor in glucose load test, which in correspondence with literature data. Also, it did not affect insulin content in blood on empty stomach, however insulin growth in an hour after glucose uptake significantly reduced. This evidences normalization of insulin reaction to glucose load and indirectly proves the reduction of insulin resistance rate. The results of the above study make it possible to draw the following conclusions. By the data of daily AT monitoring, Indopress in daily dose 2.5 mg effectively reduce it within 24 hrs and prevents rise of both systolic and diastolic AT in the morning. Under its effect true reduction of variability of SAT and DAT in day and at night as well as normalization of initially broken AT circumdaily rhythm are noted. Therapy with Indopress facilitates true reduction of APF activity in blood plasma and levels it growth in the morning (from 6 am to 9 am), catecholamines content in blood remaining unchanged. Indopress neither essentially affects glucose content on empty stomach nor with glucose tolerance test but facilitates normalization of insulin reaction to glucose load, which in whole proves the possibility of its use in patients with carbohydrate metabolism disorder with no risk of disorder aggravation.
